There is nothing to understand about such customs. This custom is usually for inauguration. But I never saw anyone breaking coconut after discharge from hospital.

What ever we do have some significance. But I dont understand why do we break a coconut in temple or in certain occasions. And I have also observed some people breaking coconut in front of hospital while discharged from hospital.
Breaking of coconut signifies breaking of one’s ego. It means the devotee or the believer is expressing his/ her commitment before the god. Breaking of a coconut is prevalent at the time of discharging a patient from a hospital signifies the devotee’s gratitude towards the god. These beliefs and rituals are in practice in southern India.

Coconut is Auspicious fruit. Its one name is Sree Phal. It is tradition and culture of India to broke it in temple and on some holy occasions.

There is a hospital near my office, when I cross from there. I see so many coconuts broken. And once my mother has also done the same while I was discharging from Hospital. When I asked her why was this, she told me that it is a system and we have to do it.

This is a custom and it is believed to break coconut when one starts something new in life. :)

Breaking a coconut in temples is quite symbolic, they represent our karma phalas or past actions, although I am not sure why they are broken outside hospitals! here is one link I found which I find reasonable enough explanation of this common ritual:

Coconut symbolises our head and if a coconut offered to god especially goddess Durga, is considered equivalent to offering one's head. Durga in all her 9 avatars is considered goddess of shakti who accepts prayers of her devotees if offered with coconut.
