Causes are too many but the recent one has been added to the list involving reckless use of mobile phones.I know of quite few accidents which were avoidable and tragically led to loss of lives!

Violation of traffic rules, negligent and rash driving cause accidents. Use of mobile phone while driving is also a traffic rule violation.
