There can be many ways in which secret reports or discussions can be leaked, esp when many persons are involved in such activities. One source is generally the tea servers or peons or sweepers who have access to such places or meetings and can overhear discussions or collect some discarded papers etc. Reporters can then piece together such leads and build a scoop or a story.

Exactly, this is what happens. That is why in armed forces, everything- every paper is considered confidential. In civil departments, the waste papers are sold as wastage. But in army, time expired record are never sold. These are burnt to ashes under proper supervision.

But there were instances where the Army papers were also leaked. In the past there were incidents of selling the army confidential reports to foreign countries.

Any papers can be leaked when the personnel are willing to do so for money. They are traitors. But adequate precaution ensures that in the ordiary course secrets are safe.

Yes sir, adequate precaution and strict implementations and severe punishment will certainly help.

When possibiity of India's partition was being discussed in Simla in a closed door meeting between Congress leaders and others ,there was a famous scoop by a Hindustan Times correspondent-Durga Das. He wired that partition was being considered and HT published it. The leaders could not deny it. Durga Das became very famous for the scoop. But he never told about his source. He wrote a book many years later namely' India from Curzon to Nehru' and wrote that feeling frustrated at no news being shared by the leaders, he bribed a tea server to only tell him the words he hears when serving tea. And he got his scoop.
