People who react for very small issues, and people who are always bothered about cleanliness around them are suffering some mental sickness
You cannot be very sure of that until a psychiatrist makes a counselling with them. But one who keeps bothering about cleanliness can be very well be a part of Obsessive Compulsive Disorder.

A known friend spends a long time at the wash basin washing his hands. The same friend checks his lock many times if it is properly locked or not. And another kind is, he imagines that he is suffering from some ailment, though the doctor says, he's not having any diseases/.These are behavioral abnormalities. A doctor/ psychologist should be consulted before jumping to conclusions.

All these symptoms are part of Obsessive Compulsive Disorder. But as you rightly mentioned you cannot jump to a conclusion without consulting a doctor/ psychiatrist.

The differences between each ailment are subtle. Commoner cannot detect them. Self medication and self diagnosis are dangerous.

Psychiatric illness are never self medicated I believe.

Bur the person who indulges in self medication doesn't know. I have seen people using crude medicines made by roadside doctors and street jugglers for Psychiatric illnesses

Yes. These are not self medicated. If patient understands sickness, he is half cured. He will be okay by auto suggestion and changing habit or attitude.

I agree. Psychiatric illness can be improved just by mere counselling.

There is very thin line in between people who like to keep things clean and those who are mentally sick and repeatedly keep cleaning or doing same things because they are not sure about themselves. This is a complex problem, which does not go easily without friends, and relatives helping them recover constantly. Also people who keep repeating the same mistakes and feel irritated if someone points it out to them come into the same category. They do not want to mend their ways.

It is much more complicated and difficult to diagnose a mental disorder since the difference between what is normal and what is abnormal is very flimsy ...like for example a very clean and tidy person might like to wash his or her hands several times , so how many times is normal ? and how many times isn't normal , is difficult to assess. A combination of several such abnormal behavioral patterns coming together may be an indicative of mental disorder, but again that should be concluded by a psychiatrist and not a layman..
Pay no mind to those who talk behind your back, it simply means that you are two steps ahead !!!
