main reason may be that these medicines are manufactured by different companies and also there are several artificial medicines also available in market therefore we must check the isi mark before purchasing any medicine.
ISI marking is not on medicines. These are subject to drug control. The checking of fake medicines is also responsibility of drug control. They do their job but as elsewhere there is corruption in this area also.
Patients are naturally not in a position to check medicines. They depend on doctors. I know that some doctors check medicines after purchase by patients. All doctors should check the medicines after a patient buys these and thereafter guide for correct dose, time and other relevant precautions.

One big problem we indians are facing now is, we are been made rats for testing of new medicines in market. Adulteration in medicine has also become very common. It is only in India human life does not have any value.
